Care for a 79-year-old elderly woman living alone, weighing 50 kg and 169 cm tall, who suffers from dementia and has recently been discharged from the hospital after her medication regimen was adjusted. The lady is oriented within her home environment and maintains temporal orientation, but she struggles to coordinate appointments and process new information effectively. Her short-term memory is weakened. Personality-wise, she is open, friendly, and cooperative when communication is good. She moves independently without aids and does not require supervision at home, but it is advisable to accompany her outside due to her dementia. She primarily needs supervision, reminders, and some assistance with dressing, hygiene, and eating. She can prepare and dress herself, but it is important to monitor the suitability of her clothing and ensure regular changes. For personal hygiene, she is mostly independent, but requires guidance and assistance as needed. She manages meals independently; it is important to motivate her to eat and ensure adequate fluid intake. She manages toileting independently. Medications are prepared, and her adherence needs to be monitored. The caregiver's duties include household management such as shopping, cooking, cleaning, and laundry care, as well as assistance with errands and accompanying her to medical appointments as agreed with the family. An important part of the care is companionship, establishing a daily routine, and engagement (spending time together, walks, short trips, for example, by bus to town, as well as activities like painting, crafting, cooking, or baking). Night duty is not required. A private room and private bathroom are available for the caregiver.
