He is often confused, forgetful, and physically weak, but is communicative. The gentleman requires assistance with all basic activities of daily living. Complete help is needed with dressing, hygiene, and toileting. He uses incontinence briefs that need changing. Assistance is required with getting up, showering, shaving, and oral hygiene. Meals need to be prepared (breakfast, lunch, dinner), and he needs help getting up to eat. It is important to monitor fluid intake and meal regularity. He sleeps a lot during the day; it is advisable to keep him awake to improve his sleep at night. Household duties are also part of the job, including cooking, shopping, cleaning, and laundry. Night duty is required, approximately twice per night, mainly to assist with toileting. Lifting is necessary. His wife also lives in the household (care level 3), is nearly blind, and cannot care for her husband. She requires minor assistance, mainly around the house, with shopping, escorting, and approximately twice a week with showering. This is physically demanding care focused on complete personal care, incontinence management, supervision for dementia, and household management for two people.
