Care for a self-sufficient 74-year-old elderly lady, 45 kg, 145 cm, who suffers from dementia, senile weakness, depression, rheumatic disease, and is in short-term care post-hospitalization. She has short-term memory impairment, occasional word-finding difficulties, is often confused and temporally disoriented in the mornings. There is a disturbed day and night rhythm, apathy, and a depressive mood. She shows no aggression or tendency to wander. She was recently widowed; her husband passed away on May 30th, which significantly affected her mental state. Currently, she cannot walk independently or move as before. The lady is urinary incontinent; incontinence aids need to be changed. She moves with the help of a walker or wheelchair; she has had multiple falls and is at risk of further falls. She is not bedridden but requires assistance with transfers from bed to wheelchair or toilet chair approximately 3-4 times a day. Light assistance is sufficient for lifting. The lady needs support with mobility, bathing, showering, dressing and undressing, walking on stairs, and daily activities. She is mostly independent when eating, sometimes requiring help and encouragement. It is necessary to monitor fluid intake, remind her to take medication, assist with doctor's visits, personal hygiene, intimate care, and toileting. Her hearing is slightly impaired, and she uses a hearing aid. She responds best to a calm and patient approach. She enjoys talking about her work, family, and dog. Currently, she is often sad, confused, and uncertain. Her favorite activities include cooking, walks, reading, watching television, visiting cafes, and spending time with her dog. She likes watching TV, company, and shared activities. The lady can also be left alone for 2-3 hours. Night duty is required multiple times depending on her current condition, especially due to weakness, confusion, and need for assistance. There is a dog in the household; its care is limited to providing water and food (no walks needed). Responsibilities also include cooking, cleaning, laundry, ironing, shopping, plant care, and general household management. Shops are located 500 to 1000 m away, and bus and train connections are also available. Her family visits daily and lives nearby. A wheelchair and walker are available, as is a car with an automatic transmission. Free time by agreement.
